If you are a new plan member, you must complete positive enrolment and provide consent to have your claims processed by Canada Life under the PSHCP. 

Positive enrolment must be completed and maintained separately on all eligible benefit plans.

If you prefer to complete your positive enrolment for the PSHCP by paper, you can download and print a paper PSHCP Positive Enrolment Form from the Forms page. 

You can also call the PSHCP Member Contact Centre to request a paper form be mailed to you. Mail back the completed form to the address indicated on the form. 

Consult your PSHCP Member Booklet for more information on Positive Enrolment and its purpose.

Positive enrolment must be completed and consent must be provided in order to have your claims processed by Canada Life under the PSHCP.

Once you have successfully completed positive enrolment for the PSHCP, make sure to create your PSHCP Member Services account through My Canada Life at WorkTM

If you already have an account for the Public Service Dental Care Plan, Pensioners' Dental Services Plan, or another plan with Canada Life, use the same email address and password to sign in.

Note: To create an account, click on Create your account then Register and use your plan number and certificate number. If you have already created an account for the PSDCP or PDSP, or another plan with Canada Life, enter the email address and password you are currently using or previously used to sign in.
