Plan information

 Your plan number is determined by your birth month or status as a member:   

  • January to March: 92111 
  • April to June: 92112 
  • July to September: 92113 
  • October to December: 92114 

The plan number for any eligible surviving dependants (spouse or common-law partner or eligible children) is 92115.

Your PDSP benefit card includes your plan number and your certificate number, which you will need to submit claims. Your dental providers will also need this information if they are submitting claims on your behalf.  

You can view, download and print a PDF copy of your PDSP benefit card by registering for or signing in to your PDSP Member Services account through  My Canada Life at WorkTM.  You can also contact Canada Life to request that a paper PDSP benefit card be sent to you by mail. 

The benefit card only includes the PDSP member’s name. The plan number and the certificate number are the same for your eligible dependant(s).

You can update or change your positive enrolment information at any time. To prevent any disruption in the processing of your claims, you are responsible for keeping your positive enrolment information up to date. 

Review your positive enrolment details carefully and regularly to ensure that all information is complete and accurate. The below chart provides you with information on who to contact to make any necessary changes. 

The Pension Centre or your pension office: 

  • your legal first and last name 
  • your date of birth 
  • your official language   
  • your category of coverage (changes or termination)  

Canada Life:  

  • your preferred name (if it differs from your legal name) 
  • your mailing address  
  • your preferred method of communication  
  • your eligible dependant(s) 
  • your banking information 

 

You can update your PDSP positive enrolment information with Canada Life digitally or by paper.   

  • Digitally – Sign in to or register for your PDSP Member Services account through My Canada Life at WorkTM. To register, you will need your dental plan number and your certificate number.   
    • If you have already created an account for the Public Service Health Care Plan, Public Service Dental Care Plan, or another plan with Canada Life, enter the email address and password you are currently using or previously used to sign in. 
  • Paper – You can download and print a paper PDSP Positive Enrolment Form from the Forms page of the PDSP Member Services website.

The PDSP costs are equally shared between the plan member and the Government of Canada. The contribution rates depend on the category of coverage selected. Only one category of coverage can be chosen.  Amendments to coverage can increase or decrease the monthly contribution.

PDSP Monthly Contribution Rates

Members of the PDSP are not eligible for the Canadian Dental Care Plan. PDSP members who choose to opt out of the PDSP can't opt back in. Exceptions may apply. For more information, visit the PDSP Rules.
