If you are a new plan member, you must complete positive enrolment and provide consent to have your dental claims processed by Canada Life under the PDSP.

Positive enrolment must be completed and maintained separately on all eligible benefit plans.

If you prefer to complete your positive enrolment for the PDSP on paper, you can download and print a paper PDSP Positive Enrolment Form from the Forms page.

You can also call the PDSP Member Contact Centre to request a paper form be mailed to you. Mail back the completed form to the address indicated on the form.

Consult your PDSP Member Booklet for more information on positive enrolment and its purpose.

Review authorizations and declarations

Positive enrolment must be completed and consent must be provided to have your claims processed by Canada Life under the PDSP.

Once you have successfully completed positive enrolment for the PDSP, make sure to create your PDSP Member Services account through My Canada Life at Work.

If you already have an account through the Public Service Health Care Plan (PSHCP), Public Service Dental Care Plan (PSDCP), or another plan with Canada Life, use the same email address and password to sign in.

To create an account, click on “Create your account” then “Register” and enter your plan number and certificate number. If you have already created an account for the PSHCP or PSDCP, enter the email address and password you are currently using or previously used to sign in.
