Public Service Health Care Plan (PSHCP) information
Your plan number is determined by your birth month:
- January to March: 52111
- April to June: 52112
- July to September: 52113
- October to December: 52114
Eligible surviving dependants (spouse or common-law partner or eligible children) plan number is 52115.
If your spouse or common-law partner is also covered under the PSHCP as a member, you may have different plan numbers depending on your month of birth. Take note if they have a different plan number to ensure claims are submitted accurately.
Your PSHCP benefit card includes your plan number and certificate number, which you will need to submit claims and for health care providers and pharmacies to send claims on your behalf.
You can access your digital PSHCP benefit card by registering or signing in to your PSHCP Member Services account through My Canada Life at WorkTM.
Your card also includes the MSH contact information for out-of-province emergency travel assistance.
You can update or change your positive enrolment information at any time. To prevent any disruption in the processing of your claims, you are responsible for keeping your personal information up to date.
Review your positive enrolment details carefully and regularly to ensure that all information is complete and accurate. The below chart provides you with information on who to contact to make any necessary changes.
Your departmental compensation office, Pay Centre or Pension Centre:
- your legal first and last name
- your date of birth
- your preferred language
- your coverage level from Supplementary Coverage to Comprehensive Coverage (and vice versa)
- change from Family to Single Coverage (or vice versa)
- change your Hospital Coverage Level I, II, III
Canada Life:
- your preferred name (if it differs from your legal name)
- your mailing address
- your preferred method of communication
- your eligible dependant(s)
- your banking information
You can update your PSHCP positive enrolment information with Canada Life digitally or by paper:
- Digitally –Sign in or register for your PSHCP Member Services account through My Canada Life at WorkTM - . To register, you will need your health plan number and your certificate number.
- If you have already created an account for the Public Service Dental Care Plan, Pensioners' Dental Care Plan, or another plan with Canada Life, enter the email address and password you are currently using or previously used to sign in.
- Paper – You can download and print a paper PSHCP Positive Enrolment Form from the Forms page of the PSHCP Member Services website.
- You can also call the PSHCP Member Contact Centre to request that a paper form be mailed to you.

- active employee versus pensioner
- single versus family coverage
- level of Hospital Provision
